Heart patients are granted concession when called by recognized hospitals for heart surgery. Cancer and thalassemia patients are granted concession for treatment and periodical check up. The element of concession is 75 per cent in Second, Sleeper, First, AC Chair Car and AC 3-tier and 50 per cent in AC 2-tier and AC First Class basic mail/express fares. Concession is granted on production of the requisite certificate from recognized hospitals. One escort with each patient is also granted same concession.

However, similar concession is not granted in Rajdhani, Shatabdi and Jan Shatabdi trains.
